Attendee Information
To Complete Registration

To ensure complete registration please follow these steps:

1. Fill out the form completely.

2. Select “Next” at the bottom of the page.

3. A verification page will appear. Review the information and select “Confirm” at the bottom of the page.

4. A payment page will appear. If you select to pay online with credit/debit card you will be re-routed to OSU’s secure payment gateway. Once you have provided the payment information, there will be a link at the bottom of the screen after payment is completed that will route you back to the registration system. You will then see a letter of confirmation on the screen. It will also be emailed to the address provided. A separate email titled "Notification of Payment" will be sent to the email address you specified in the credit card portion of the registration process.

5. If you select “Invoice” an invoice will be emailed to the address provided within 5-7 business days. A letter of confirmation will appear on the screen and will be delivered to your inbox as well.

6. If you are part of Oregon State University, you may select "Index". A box will pop up and you will provide your index and activity code (if applicable). A letter of confirmation will appear on the screen and will be delivered to your inbox as well.


Please note that the letter of confirmation gets emailed to the email address you provided at the top of this form.

Linen

Regular registration does include lodging as noted above.

Please note that all attendees will need to bring their own linens including either sheets/blankets or a sleeping bag, pillow and towels.

If you are unable to bring your own linens, basic linens can be provided for an additional $10.00.

Poster Session and Pop-Up Talk

All attendees are invited to bring a poster for the poster session. Please indicate below if you will be bringing a poster with you.

Cancellation and Refund Policy

Cancellation requests need to be made in writing no later than 5:00 p.m. (PST) Friday, March 27, 2015 in order to receive a refund of your registration fee(minus a $35.00 administrative fee). Requests made after 5:00 p.m. (PST) Friday, March 27, 2015 will not receive a refund. Substitutions may be allowed.
